National ID or (Civil Status ID + Nationality Certificate) - Original + Color Copy.
Residence Card - Original + Color Copy.
Two (2) recent personal photographs for both parties.
Support of Service Continuation addressed to the 'College of Pharmacy'.
Legal Notice: Personal presence of both the Student and the Guarantor is mandatory.
